Project Make Lemonade Pty Ltd (ABN 29 634 590 373) — NDIS Provider Number: 4050061674 — Last updated: August 2026

Project Make Lemonade is committed to protecting the privacy, dignity and confidentiality of the participants, families, workers and stakeholders we work with. This policy explains how we collect, use, store and share personal information, and it reflects our obligations under the Privacy Act 1988 (Cth), the Australian Privacy Principles, the NDIS Practice Standards and the NDIS Code
of Conduct.

Our approach to privacy
We handle personal information in line with these principles:

  • Respect and dignity — your privacy and identity are respected in every interaction

  • Transparency — we'll always explain how your information is collected, used and shared

  • Consent and choice — we ask before sharing your information, and support you to make informed decisions

  • Minimum necessary information — we only collect what we genuinely need to deliver your supports safely

  • Security — your information is protected with appropriate safeguards

  • Access and accuracy — you can ask to see your information or have it corrected

  • Cultural safety — we handle information with respect for your cultural identity and communication needs

What information we collect
Depending on the supports you receive, we may collect:

  • Your name, date of birth, address and contact details

  • Your NDIS plan details, funding categories and goals

  • Health, disability or support-related information relevant to your care

  • Emergency contact and representative details

  • Feedback, incident or complaint information

Where possible, we collect this directly from you. We may also collect it from
a support coordinator, guardian, representative or health professional,
where appropriate.

How we use your information
We use your information to plan and deliver your supports, coordinate with other providers involved in your care, respond to safety or incident concerns, meet our NDIS and legal reporting obligations, and improve our services. We only access information relevant to the person's role, and we don't use it for anything unrelated without your consent.

Who we share it with
We may share information with your support coordinator, guardian or nominee (where authorised), other providers involved in your care (with consent), and the NDIA or NDIS Quality and Safeguards Commission where required by law. We never sell your information or share it for marketing.

Consent
Where we need your consent to share information, we'll make sure you understand what's being shared, who with, and why — and that you can change your mind at any time, subject to legal requirements. There are some situations where we may need to share information without consent, such as mandatory reporting or safeguarding obligations.

Keeping your information secure
We store information in secure, access-controlled systems, and paper records (where used) are kept in locked storage. Access is limited to staff who need it to do their job. Information no longer required is securely destroyed or
de-identified in line with our records management obligations.

Your rights
You can ask to see the personal information we hold about you, ask us to correct anything that's wrong or out of date, and raise a concern if you think your information has been mishandled.

Privacy complaints
If you're unhappy with how we've handled your personal information, contact us using the details below. You can also contact the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au, or the NDIS Quality and Safeguards Commission on 1800 035 544.
